Returning patients: If it has been over one year since your last visit, you will be asked to complete updated paperwork to ensure we have the most accurate and current information on file.

What to Bring:

To help us get you seen quickly, please bring:

• A valid government issued ID or passport
• Insurance card (if applicable)
• List of current medications and allergies
• Payment method for any out-of-pocket costs
• If Applicable: Legal documentation if you are bringing a minor or another individual for whom you are authorized to make medical decisions (e.g., guardianship papers, medical power of attorney, or custody documentation)
